What is responsible for these large nodules on the leg? What disease is this condition most associated with?

  1. This is hyperthyroidism with thyroid dermopathy in a classic location on the anterior shins. Note that infiltrated plaques extend to the calf and are hyperkeratotic. An isolated nodule is also present on the dorsum of the foot.

Thyroid dermopathy is usually a later manifestation of thyroid disease. Hypothyroidism with onset in adulthood causes a generalized decrease in metabolism, with slowed heart rate, diminished oxygen consumption, and deposition of glycosaminoglycans in intracellular spaces, particularly in skin and muscle.
