This is pharyngitis with marked white exudates on the tonsils of an individual with Epstein-Barr virus (EBV) infection. Tonsillar exudates are commonly seen in acute EBV infection. It often begins with malaise, headache, and fevers before development of the more specific signs of exudative pharyngitis and posterior cervical lymph node enlargement. Treatment is largely supportive; however, some patients may require IV fluids and pain medication. A dose of oral or parenteral steroid can be considered to reduce tonsillar enlargement.
